The first and only over-the-counter product for weight loss approved by the Food and Drug Administration will be available Friday, June 15.
Orlistat, known by the brand name Alli, works by decreasing the amount of fat absorbed by the body. It is the OTC version of Xenical, a prescription weight loss pill. The good news: Orlistat has been tested and the prescription version has been used since 1999.
Last fall Dr. James Anderson, head of the UK College of Medicine Metabolic Research Group, and his colleagues examined the effects of OTC strength (60 mg) orlistat on mildly to moderately overweight individuals. The study was the first of its kind. Previously, the drug’s effects had only been studied in obese individuals. Study participants took either orlistat or a placebo three times daily with meals for 16 weeks. Results of that study showed those taking OTC-strength orlistat did lose more weight than those taking the placebo.
“Our research showed that people taking orlistat and following low-fat diets lost almost five percent of their initial body weight, about seven to15 pounds, over four months,” Anderson said. “While two to four pounds a month isn’t dramatic, steady weight loss of this amount can have major health benefits. For example, the reduction in LDL-cholesterol, the bad-guy cholesterol, of 10 percent can reduce risk of heart attack by 20 percent.”
Any successful dieter knows that long-term weight loss is about lifestyle changes not quick fixes. While taking Alli may help you lose weight, it won’t do all of the work for you. Anderson stresses a healthy diet and exercise plan are absolutely necessary to lose the weight and keep it off.
“This is the first over-the-counter medicine that has proven effectiveness. It is my hope that people will take one capsule before each regular meal, breakfast, lunch, and supper, and alter their fat and calorie intake,” Anderson said. “If they commit to exercise six days a week, most people can lose weight steadily. All of us are in this for the long haul and need to keep up healthy behaviors, not for days or weeks, but for months and years. Doing regular physical activity and making good food choices will help us be trimmer and give us more energy.”

We must take it along with diet and exercise to be able to successfully achieve our weight loss goals. i takes about 10 days for alli to start working properly, after this any fatty foods you eat the greese goes straight through you like a porsche, iv used alli a few times and allways had good results, but when you stop taking the pills you put on weight fast its like your body stores the fat its missed but all in all alli is the only slimming aid that realy works.
Its a good content but I think some slimming pills like uniquehoodia, phen375 are good some not, Its better if we do intake good diet, intensive exercise and fibre rich supplements that helps to elevate metabolism what you guys think?

I’m on my fifth day of Alli… I have eaten healthy ish all week but today I thought I would risk the chip shop – jumbo sausage & chips..which was very greasy and very fatty, (it tasted awful to be honest – not sure if this is something to do with the tablebs) BUT guess what, several hours has past since the fatty meal, plus a mars bar for pudding.. and.. shock horror, NO side affects.. Alli is just a mind game – no fat burning going on here ! Waste of money i’m afraid to say.

I found that almost all OTC pills are a scam, however, there are some really good ones out there. You just have to look for something that seems legit. No wild claims, just a humble diet pill with good packaging. Get one that encourages exercise and a healthy diet.
A lot of OTC diet pills are pure scam. Often just a bunch of dried up berries turned into a powder and mixed with caffeine. The important part is to look for a diet pill that is going to help with multiple aspects of weight off. Also, look for one that encourages exercise and dieting, not a miracle cure-all.
However, its important to expose the diet pills that are a scam and diet pills that really work.
I am not really convinced yet. Have there been long term studies published so far?
How exactly is fat blocked from being absorbed? What about essential fatty acids? Will they also be excluded from absorption?

Not sure whether Alli is really worth the money or not. I’ve been following the Slimming World diet for 6 months and lost nearly 4 stone. I can lose 3lb some weeks and never be hungry – it’s worth checking out. It’s a low-ish fat plan so I thought I’d try Alli to ‘boost’ my weight loss but put on a pound in week one believe it or not! If you’re eating low enough fat to not trigger the side effects (I haven’t) then I do wonder about the point of Alli. If you use it to make up for unhealthy eating, you pay for it – but if you’re eating healthily anyway – you don’t really need it. I’m wishing I’d saved my £30. People shouldn’t be using it to counteract a binge – it won’t work, which is why some are saying that they put the weight back on when stopping Alli. I’m surprised that people taking it aren’t aware of side effects – you really should be reading the leaflet that comes with the tablets.

I have been taking alli tablets and reducing fat and calorie intake quite drastically down to 700 calories a day and taking alli 3 times a day, I have just wieghed myself I was 11st 11lb I am now 11st 2lb, I have never lost this much wieght on any diet which leads me to believe alli’s working, I also have’nt had any of the nasty side effects either probebly the complete oposite only had 1 poo in a week which had no oil in it at all which I thought was strange good luck all you dieters!

I too love Alli, and I think we have all had the dreaded “accident” while taking it. I’m like you though, I don’t want to stop taking it because it’s worked better than anything else I’ve tried.
The oily spotting is going to happen every now and then, so I put a Poise pad (which is used for urinary incontinence) just in case I have a leak. They are smaller than depends and are basically an oversized maxi pad. You can’t see them through your clothing and they give you some piece of mind in case something happens before you can get to a bathroom. In other words, it’s a barrier that will keep stuff from running down your leg. Just a bit of advice. It’s worked for me and now I don’t have to wear shirts that cover my butt all the time!

Okay, I’m going to be candid without being gross. I take Alli and it’s been great. There are times when I’m in a situation where I can’t avoid eating more fat than I want, and yes if you do that, you can have some leakage. If I know I’m going to an event where I may eat a little more fat, I put a Poise pad in my underwear just to be in the safe side. i have taken alli for about 2 months now and, although i have experienced the dreaded oil poopies, they are not so annoying as to cause me to discontinue the medication. i have never shat myself–whether at home or in public–and dont expect to ever have that exciting experience, not even when i have had a very high fat meal. i would have to say that the most important aspect of dealing with the after effects of alli is to have copious amnts of commet and a stiff bristled toilet brush on hand available after each episode of unfortunate oil poopy. it does tend to leave an orange ring around the toilet, but happily, none of my knickers have been harmed in any way.
How much fat is blocked per pill of Alli? Am i correct to say that if one meal contains 15 grams of fat, one Alli pill will block 30% of it, which works out to be 4.5 grams blocked?

You need to seriously adjust the way you eat. This way the pill can be taken, as recommended, 3 times a day.
Oil may still appear in your stool but with a meal under 15 grams of fat, you SHOULD NOT have uncontrollable diarrhea!
If you are shitting your pants after every meal…you need to think long and hard about what you are putting in your mouth!

I’ve lost 2 lbs so far and have been eating better and exercising 3 days a week.

Used Alli for about a week now, not obese just looking to lose a few baby lbs. OMG !! Oil ! Better out than in though ! I’ve learned, I only take it when I’m home. I had an incident of oil spotting ! If I had been at work I’d have been pissed !! Luckly I was home. I only take them at night, or in the afternoon after work. Those afraid of side effects should try that. Good Luck.

YES! I also noticed the constipation is gone! I know of several people on similar prescribed meds as I am and one of the huge side effects is constipation. Within 3 days of starting Alli, I immediately noticed a change in bowel movements…for the better! I believe since I was very constipated prior to starting Alli, the fat that is eliminated with the bowel movement, makes me ‘regular’…hence no horrible or embarrassing side effects!!!
I’ve been constipated all my life. After starting Alli and eating a meal with some fat twice a day, I am no longer constipated. No longer taking laxatives or stool softners which never worked. I go to the bathroom regularly without diarhea.
I’m a “middle aged” male who started Alli on February 2, 2008. I was on it for 9 weeks and lost 27 lbs – going from 193 to 166. Since then I’ve fluctuated from 163 to 170 – today at 168. It is a miracle drug. The bonus was the increased energy. Yes, I work out on an elliptical four or five times a week and I try not to be too stupid about what I eat. But those were pounds I’d tried to lose for 5 years.
I think the success to the program is that the “treatment effects” force you to eat right or else. It’s like having a 24 hour nutritionist on staff. If you don’t follow the guidelines, you might pay for it, so it’s better to just follow the program. And besides, if you don’t follow the guidelines, why waste money on the pill? The other reason it worked for me is, as one who loves to eat, I wasn’t needing to watch calories and food intake so much, only fat grams. So as long as I ate the right things – low fat – the quantity was not as important.
I never thought I’d try a diet pill but I’m glad I did and I can’t recommend it highly enough.
I first heard of Alli when my brother started taking it a year ago. He probably only used it for about two weeks and I personally didn’t see any results. Well about five months ago I gave birth and while pregnant I didn’t gain any weight. But, as soon as I stopped breast feeding I gained a horrifying 20 pounds. I went out and bought a bunch of exercise equipment even a pilates kit but nothing seem to help. I finally decided after I reached the 170 mark and all my favorite jeans weren’t fitting that I was going to try a diet pill. I went to CVS and asked the Pharmacist opinion on pills and he told me that Alli was the only FDA approved one. So even though it didn’t work for my brother I decided to spend the 60 dollars for the starter kit and I bought a digital scale. Needless to say I started taking Alli on Feb. 23, 2009 and I weighted in at 170.0 Ibs and today which is Feb. 27, 2009 I weigh in at 165.6 Ibs. I haven’t been exercising at all because I am a full time student and mother but I do plan to start. I have been on 1100 calories a day and eating only 10 grams of fat a meal (To avoid the bathroom situation which lucky I haven’t encountered yet.) I snack on Special K Chocolate bits with skim milk between meals, and I bake everything. Also, I cut out soda and bought a bunch of bottled water and Crystal light to go packs. So after 4 days I can honestly say I am having great results with Alli and even learned some self control.

I am a 48 year old woman, 5’2″, and my starting weight was 192 lbs. In early Feb. I decided I needed to do something about my weight…..for my own good, so I thought I’d try Alli. I am now down to 173! Sure, I am eating right (mostly) and exercising, but I do believe Alli has helped. I’ve dealt with the cause effects a little bit and it hasn’t been too bad. All you have to do is just remember to not eat so much fat. It’s not that difficult. I am overall pleased with Alli and would recommend it to anyone who is really ready. If you’re not willing to give up the fat and calories then you aren’t ready for Alli. Good luck to everyone.

Can you tell me how many hours after a fatty meal do the side effects start (the oil discharge) and how many hours does it last? I would like to use Alli at night only and was wondering if I am safe during the day if I just take 1 pill at supper time, does the discharge occur during sleep time in that case or is it immediatly after the meal?

I think the major reason why people give up on alli is their misunderstanding of how the drug works. Alli is NOT a hunger suppressant. It never promises that you can eat whatever you want and control your appetite. It simply promises to block a percentage (25%) of the fat your body absorbs. Common sense would then lead one to realize that if your body is not absorbing that fat, it has to go SOMEWHERE. Whatever the body DOES NOT USE is eliminated as WASTE… hence, those of you who refuse to accept the fat that you need to cut down the fat intake dramatically will find yourselves in a very bad position eg. hovering over the toilet with oily crap running down your leg at the mall. Keep in mind as well, that will power is a major key to the success of this pill. YOU MUST be WILLING to NOT eat the FAT! Why? Alli IS NOT a hunger suppressant. YOU must control your own hunger. Will power, exercise, right choices, low fat diet and plain old common sense will allow you to be successful at using Alli without side effects of any kind.

Hi, well I tried the Alli pills and I will not continue! I found a good gum that stops me feeling hungry. It’s cheap and I have lost 55Lb now, I wont go back to the Alli pills due to the toilet problems.
I have been taking Alli for one week and have not exceeded the 15 grams of fat per meal (I am very careful not too) but I have persistent uncomfortable stomach/gastro issues such as gas (and yes, I am afraid to fart sometimes in case of the unwanted side effect, which I have not experienced as of yet).
Here is how I feel at every meal:
bloated/gassy
rumbling in my stomach/gastro area
foul smelling gas and bowel movements
heartburn and I have not heard anyone yet complain about that, but I am getting it and never have before???!
I am a 42 year old male, who is 6’2″, 223lbs and lost about 7 pounds over a month ago and so tried to add this on to see if I could drop another 7 in the next month. I do excercise, which gets easier as you get thinner and will see this through a month to see if it makes a meaningful difference, if not, then I cannot put up with this discomfort. I believe in no pain no gain, so they should use the slogan pain = no gain (LOL). Trying to stay upbeat about losing weight and getting healthier.

I loooove ALLI. There are side effects. Loosing wieght takes time, hard work, and is not easy! Some people have a problem with this but I welcome small daily challenges like unwanted visitors like oils or even solids running down my leg. Its less painfull than running on the treadmill! Some people are afraid of people thinking they smell or are dirty (due to spotting through the paints/mini skirt) but I say your gross if your FAT!

I have been taking the alli pill for a month. I try and stay within the fat limitations, but sometimes cheat. I have lossed 16 pounds in one month. I get the side effects when I eat more than what is allowed on the fat intake. If you follow the fat intake you will lose weight and not have the side effects. That is just what I have had so far. I think this is a good healthy way to lose weight.
I have been taking alli for about a week now. I have noticed a significant amount of the “oily spotting” as warned by the manufacturer. I even had a embarassing incident where the accumulated oil actually built up enough for a little to run
. My domestic partner also has been unable to tolerate sex lately, the oily fluids have a tendancy to cause his condom to break in less than a minute. If I don’t start seeing results soon, I will discontiue use. Its side effects are just too drastic.

Following is another post from the old ARNA board on Yahoo, where I discussed the Acomplia results a year ago, in response to a skeptic about Lorcaserin. I think there are some much more effective diet drugs on the horizon.
“You’re too pessimistic about ARNA. Everything is proceeding reasonably well, the only current delay is getting the OK from the FDA to move ahead with the APD125 phase 2. Lorcaserin is on track to start phase 3 this year, as are the two partnered programs, as is the anticoagulant program. Sanofi’s rimonabant, the weight loss therapy that ought to pose the greatest competition to Lorcaserin, is held up at the FDA and my bet it’s a safety issue, given all the publicity regarding Vioxx, Tysabri, and others. I think it’s probably the depression signal seen in the the trials. Here’s how I think about it….the THC in marijuana is a CB1 agonist, and causes euphoria (the “high”) and the “munchies”. Rimonabant is a CB1 antagonist and promotes less food intake (weight loss) and apparently depression in some sensitive individuals. In the recent, published study, there was a depression rate of 3% in the patients given 20 mg of rimonabant daily. I think that’s too high to be acceptable, although it’s possible it will eventually be marketed in the US with a black box warning. Other competition includes the companies developing PYY3-36, AMLN’s Symlin + Leptin combination looks exciting in pre-clinical studies, and certainly others as well.
Regarding the current stock price, I’m not worried. The market moves up and down in the short term due to anxieties and enthusiasms of market timers and daytraders and institutions that have to show quarterly results, but long term changes are due to fundamentals and these are more predictable. …. If one were to buy and hold ARNA now, I am fairly confident it would be an excellent decision in the long term.”

I’ve commented on the likelihood of success for an OTC lipase blocker previously….I think people will try it, indeed, a lot of people will try it. However few will stay on the drug because of the side effects. Following is a light-hearted, yet thus-far witheringly accurate commentary I posted on the Yahoo ARNA message board roughly a couple of years ago:
There’s no question Xenical can effectively block fat absorption and lead to modest weight loss with long term use. The “but”, however (note that I manfully resisted the extra “t”), is that the fat which isn’t absorbed passes into the large intestine, where much of it, but not all of it, is cheerfully utilized by bacteria. The bacteria, in turn, make lots of gas. The end result of all this (har) is a lot of gas that smells like spoiled meat. Regular Xenical users are not welcome around the watercooler at work. Now, the stinky gas problem can be ameliorated somewhat with strict adherence to a very low fat diet, but if the patient could do that in the first place, they wouldn’t need Xenical. So while there’s no doubt OTC Xenical will eventually have some market share, it will be a small, outdoors market share……
