A colorectal tumour, for example, can be removed completely from a patient but then pop up elsewhere in the body in another guise.<\/p>\n<p>Researchers in Spain believe they\u2019re on the right track to stop this type of cancer from spreading.<\/p>\n<p><strong>Deadly wanderers\u00a0<\/strong><\/p>\n<p>A colorectal tumour can be removed from the gut with surgery and chemotherapy and some patients will never again get the disease.<\/p>\n<p>Yet this type of cancer is capable of a shape-shifting trick. A small number of cells often leave the main tumour in the gut and can seed a new cancer in vital organs such as the liver and the lungs\u00a0\u2013\u00a0a process known as metastasis.<\/p>\n<p>\u2018This is a reason why patients die of colorectal cancer,\u2019 said Dr Eduard Batlle, head of the cancer-science programme at the Institute for Research in Biomedicine in Barcelona, Spain. \u2018Mestastasis is very difficult to treat.\u2019<\/p>\n<p>Colorectal cancer is the second-leading cause of cancer deaths in the EU, where more than 156 000 people are\u00a0<a href=\"https:\/\/ecis.jrc.ec.europa.eu\/pdf\/Colorectal_cancer_factsheet-Mar_2021.pdf\">estimated<\/a>\u00a0to have died from the disease in 2020. One in 22 men and one in 35 women are at risk of developing this illness during their lifetimes, with a total of more than 340 000 new cases in EU countries in 2020 alone.<\/p>\n<p>Such statistics have helped prompt the EU to make the fight against cancer a top policy priority. Two broad initiatives\u00a0\u2013\u00a0the\u00a0<a href=\"https:\/\/research-and-innovation.ec.europa.eu\/funding\/funding-opportunities\/funding-programmes-and-open-calls\/horizon-europe\/eu-missions-horizon-europe\/eu-mission-cancer_en\">EU Mission on Cancer<\/a>\u00a0and\u00a0<a href=\"https:\/\/ec.europa.eu\/commission\/presscorner\/detail\/en\/IP_21_342\">Europe\u2019s Beating Cancer Plan<\/a>\u00a0\u2013\u00a0aim to make major inroads this decade and highlight the political commitment in Europe to the matter.<\/p>\n<p>When a patient comes to hospital with a colorectal tumour, it\u2019s impossible to know whether a small number of rogue cells have already journeyed around the body and set up tiny cancer timebombs in the liver or lungs.<\/p>\n<p>This is no minor matter. Up to 40% of patients who have their colorectal cancer surgically removed will suffer a relapse elsewhere in their body.<\/p>\n<p>Batlle leads a research project that has received EU funding to find out which cells exit the main tumour and how they might be eliminated. The initiative, called\u00a0<a href=\"https:\/\/cordis.europa.eu\/project\/id\/884623\">ResidualCRC<\/a>, is for five years until end-August 2025.<\/p>\n<p><strong>Revolutionary cure<\/strong><\/p>\n<p>Batlle\u2019s laboratory made surprising discoveries about the wandering cancer cells and his team believes it has a strategy to remove the deadly seeds.<\/p>\n<p>\u2018We identified a previously unknown cell type that is capable of migrating out of the primary tumour,\u2019 he said.<\/p>\n<p>A tumour in the colon may have lots of these cells or just a limited number of them.<\/p>\n<p>Batlle thinks they can be rooted out using a revolutionary new treatment: immunotherapy. It involves \u201ccheckpoint inhibitors\u201d, which block proteins that prevent the immune system from fighting off cancer cells.<\/p>\n<p>Normally, a few rogue cancer cells are tackled by T cells, an arm of the immune system that specialises in this task.<\/p>\n<p>But cancer cells can fight back. For example, they make a protein that tells T cells to stand down, giving the cancer the opportunity to go on spreading.<\/p>\n<p>\u2018They develop ways to escape the immune system,\u2019 said Batlle.<\/p>\n<p>Checkpoint inhibitors essentially turn patients\u2019 T cells back on so that they become hyper-aware of cancer cells. This encourages the T cells to find and kill the hidden cancer timebombs.<\/p>\n<p><strong>Seek and destroy<\/strong><\/p>\n<p>While T cells face a tough battle against the tumour itself, which is hard to penetrate, it can be removed as usual with surgery and chemotherapy.<\/p>\n<p>Batlle plans to treat colorectal cancer patients with checkpoint inhibitors even before a surgeon removes the main tumour.<\/p>\n<p>The hope is that patients will be cured of their cancer and be protected from metastasis.<\/p>\n<p>The Barcelona lab worked on mice with colorectal cancer and used immunotherapy to treat any wandering cancer cells in the rodents.<\/p>\n<p>\u2018This gave us a chance to investigate a phase of the disease that is invisible in humans,\u2019 said Batlle. \u2018We showed that, at the micro-metastatic stage of the disease, these cells are vulnerable to immunotherapy.\u2019<\/p>\n<p>The goal is for the same treatment to work for cancer patients.<\/p>\n<p><strong>Cancer in a dish<\/strong><\/p>\n<p>Another way to investigate cancer outside a patient\u2019s body is to grow mini organs, or organoids.<\/p>\n<p>Batlle has collaborated with Dr Xavier Trepat at the Institute for Bioengineering of Catalonia, or IBEC, in Spain. Trepat received patient cancer cells from Batlle\u2019s lab and used them to grow organoids in a dish.<\/p>\n<p>\u2018We can test several drugs on a tumour from one patient,\u2019 said Trepat. \u2018That way you are not doing trial and error in the patient but on tissue in the lab grown from the patient.\u2019<\/p>\n<p>He has a particular interest in how physics and physical forces can influence biology. For example, tumours are often stiffer than surrounding tissue and are physically different.<\/p>\n<p>\u2018We build these intestinal organoids and study how the tissues adopt the shape that they do,\u2019 said Trepat, who is a group leader at IBEC.<\/p>\n<p><strong>Biological robots<\/strong><\/p>\n<p>This research is part of a separate EU-funded project to learn more about a type of cell that could allow for future biological robots.<\/p>\n<p>Called\u00a0<a href=\"https:\/\/cordis.europa.eu\/project\/id\/883739\">EpiFold<\/a>, the five-year project runs until the end of 2025.<\/p>\n<p>The cells in question belong to the epithelium, which is a thin tissue that covers all internal and external surfaces of the body. They play a key role in protecting the body and controlling entry into tissues and organs.<\/p>\n<p>EpiFold aims to use 3-D organoids to understand how this protective layer takes on its proper shape.<\/p>\n<p>Discoveries could inspire new biohybrid devices, creating microrobots surrounded by living epithelial tissue.<\/p>\n<p>Someday such biological robots could self-repair, as living tissue does, or target the tough defences of tumours by drilling inside and releasing a cancer-killing cargo.<\/p>\n<p>Meanwhile, colorectal cancer will remain a complex and deadly disease.<\/p>\n<p>Diagnosing and treating patients more effectively will require not just medical doctors but also many types of scientists to improve understanding of the disease.<\/p>\n<p>Researchers like Batlle and Trepat are helping show the way ahead for dealing with colorectal cancer and its lethal spread.<\/p>\n<p>\u2018We are now discussing our strategy with clinicians and we are hopeful that it will benefit patients,\u2019 said Batlle.<\/p>\n<p><em>Research in this article was funded by the EU via the European Research Council (ERC). <\/em><\/p>\n<p><strong>BEATING CANCER<\/strong><\/p>\n<p>Europe accounts for a quarter of the world\u2019s cancer cases while representing 10% of the global population.\u00a0In the EU in 2020, 2.7 million people were diagnosed with cancer and another 1.3 million people died from the disease.<\/p>\n<p>The\u00a0<a href=\"https:\/\/research-and-innovation.ec.europa.eu\/funding\/funding-opportunities\/funding-programmes-and-open-calls\/horizon-europe\/eu-missions-horizon-europe\/eu-mission-cancer_en\">EU Mission on Cancer<\/a>\u00a0aims to improve the lives of more than 3 million people by 2030 through a range of actions including treatment. As a major component of the EU\u2019s investment in cancer research and innovation, the Mission will deepen understanding of the disease, focus on prevention and earlier diagnosis and improve patients\u2019 quality of life during and after their treatment.<\/p>\n<p>Together with the Mission,\u00a0<a href=\"https:\/\/ec.europa.eu\/commission\/presscorner\/detail\/en\/IP_21_342\">Europe\u2019s Beating Cancer Plan<\/a>\u00a0is tackling the entire disease pathway from prevention to quality of life.
