Only Experimental Subject Who Matters"},"content":{"rendered":"<p><em>We have come to expect that clinical trials require thousands of participants, tracked over years or decades. \u00a0Even for measures of clear long-term value like exercise and vitamin D, the trends have to be wrenched from the scatter with statistical vicegrips. \u00a0The message in this situation is that individual responses to any intervention vary widely, and the individual variation is far larger than the average effect. \u00a0Does this suggest an individual strategy for your personal health? \u00a0One man\u2019s food is another man\u2019s poison. \u00a0You are not an average. \u00a0There\u2019s enormous potential benefit if you can figure out which side of the curve you fall on. \u00a0Of the many supplements and diets and practices that are beneficial to some people but not others, which is most helpful to <strong>you<\/strong>?<\/em><\/p>\n<p><a href=\"https:\/\/futurism.com\/personalized-medicine-may-do-more-to-treat-rather-than-prevent-chronic-diseases\/\">Headline: <strong>Soon, Medication Will be Custom Tailored to Your Specific Genetics<\/strong><\/a><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignleft\" src=\"http:\/\/img.pandawhale.com\/93628-all-life-is-an-experiment-pMnK.jpeg\" width=\"328\" height=\"326\" \/><\/p>\n<p>Everyone agrees that individualized medicine is the wave of the future. \u00a0You don\u2019t have to wait for a gene map that tells you exactly the right treatment for your personal gene combinations. \u00a0With a little patience and discipline (ok\u2014maybe a lot of discipline), you can find out for yourself. \u00a0What you need is a system for trying many different ideas\u2014the more diverse the better\u2014and a notebook or spreadsheet for recording what you notice.<\/p>\n<p>There are many conditions at which Western medicine excels. \u00a0Vaccines have wiped out polio, which left my father with a right leg 4 inches shorter than his left. \u00a0If you have appendicitis, don\u2019t hesitate to get an appendectomy, and if you contract malaria, thank science for quinine. \u00a0\u00a0<\/p>\n<p>But there remain many conditions for which Western medicine has no answer, and standard practice is to mask symptoms with temporary expedients. \u00a0It means only that medical science has not identified a cure that works for <strong><em>everyone<\/em><\/strong>; there may well be something available that can cure <strong><em>you<\/em><\/strong>.<\/p>\n<p>All of \u201cevidence-based\u201d medicine is derived from studies of many people, usually thousands of people, sometimes millions of people. \u00a0The usual situation is that individual responses vary all over the map, and what is reported is the average. \u00a0<\/p>\n<p>If there\u2019s an absolute cure, no side-effects, no recurrence, but it only works in 20% of the subjects, you won\u2019t even hear about it. \u00a0If there\u2019s a cure that requires discipline\u2014for example, a rigorous exercise program or a severely restricted diet&#8211;you\u2019ll never hear about it even if it works for everyone, because the study will be undermined by \u201ccompliance issues\u201d.<\/p>\n<p>Of course, let\u2019s learn all we can learn from large-scale studies. \u00a0But don\u2019t let\u2019s stop there. \u00a0The one experimental subject you care about is the one over whom you have the most control. \u00a0You are not an average, but you are knowable\u2014your tolerances and your limits, your preferences, your individual and highly specific response to a medication or a diet or a new rhythm of sleeping and waking, of discipline and free play, of working and working out.<\/p>\n<p>The procedure is perfectly straightforward and common sensical, though few people are doing it. \u00a0It is the essence of the scientific method:<\/p>\n<ul>\n<li style=\"font-weight: 400\">Choose one condition to focus on. \u00a0Prioritize what will have the greatest impact on your wellbeing, but also consider what has clear symptoms you can feel or measure. \u00a0Start with something about which you feel open-minded and optimistic\u2014you can advance later to chronic conditions for which you may have abandoned hope.<\/li>\n<li style=\"font-weight: 400\">Choose a treatment or change in life habits that you think has a chance of addressing that condition. \u00a0(Guidance for this step below.)<\/li>\n<li style=\"font-weight: 400\">Decide how long is a fair test. \u00a0Naturally, an approach that offers results in with a few days\u00a0is easier to test than something that you suspect will take a year. \u00a0(We\u2019ll use \u201ctwo weeks\u201d as an example.)<\/li>\n<li style=\"font-weight: 400\">Choose a time when you expect a routine that is typical for you. \u00a0Better not to start at a time when you\u2019re traveling or beginning a new job or a new relationship.<\/li>\n<li style=\"font-weight: 400\">Begin keeping records. \u00a0Every evening without fail, record a number that codes how well you\u2019re doing with this condition. \u00a0Add a few words of description if you like. \u00a0Keep your record in a diary, a notebook, or (if you\u2019re comfortable with them) in a spreadsheet. \u00a0Begin with a two week reference period, life as usual.<\/li>\n<li style=\"font-weight: 400\">Then begin your first treatment period. \u00a0Keep records for another two weeks.<\/li>\n<li style=\"font-weight: 400\">Only at the end of the first four weeks, look back at your daily records. \u00a0Can you see any difference between the reference period and the test period? \u00a0It may be clear at this point that this treatment isn\u2019t working, and it\u2019s time to try something else. \u00a0But unless you\u2019re pretty sure, give it at least one more trial period: two weeks off and two weeks on.<\/li>\n<li style=\"font-weight: 400\">At this point, you have a decision to make. \u00a0You have recorded your subjective judgments, and now you step into the role of objective scientist\u00a0to make the decision based on your data. \u00a0Was there a clear difference between the test periods and the control periods? \u00a0(Secondarily, ask, \u201cis this something that it is easy to continue doing?\u201d) You\u2019re not desperate\u2014there are lots of other things you can try. \u00a0Make a decision to\n<ul>\n<li style=\"font-weight: 400\">Drop this idea for now and try something else.<\/li>\n<li style=\"font-weight: 400\">Keep doing this, and add a new treatment.<\/li>\n<li style=\"font-weight: 400\">Keep doing what you\u2019re doing\u2014it looks as though it solves the issue completely.<\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\">Repeat until you are where you want to be.<\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p><strong>How do you know if it\u2019s working?<\/strong><\/p>\n<p>Some kinds of feedback are easier than others. \u00a0The most difficult concern your long-term risk of getting cancer or heart disease, but even here you may be able to find surrogate measures that provide a good indication of what is helping.<\/p>\n<ul>\n<li style=\"font-weight: 400\">The easiest case is objectively measurable. \u00a0If you are interested in losing weight or lowering blood sugar or increasing sprint speed or the number of pushups you can do, it\u2019s obvious what to measure. \u00a0(There\u2019s emerging technology for checking blood sugar frequently, without finger pricks, available from several sources [<a href=\"https:\/\/www.dexcom.com\/get-started-cgm\">Dexcom<\/a>, <a href=\"https:\/\/ihealthlabs.com\/glucometer\/wireless-smart-gluco-monitoring-system\/\">iHealth<\/a>, <a href=\"http:\/\/www.news.com.au\/lifestyle\/health\/health-problems\/new-device-for-diabetes-eliminates-the-need-for-painful-finger-pricking\/news-story\/539d9044155f4866ce7e41b770844001\">Abbotts<\/a>, <a href=\"http:\/\/www.medicalexpo.com\/medical-manufacturer\/non-invasive-blood-glucose-meter-13523.html\">review<\/a>].) \u00a0<\/li>\n<li style=\"font-weight: 400\">More subtle are subjective feelings, including chronic pain, congestion, stiffness, anxiety, wellbeing, engagement, enthusiasm, mental focus and physical energy. \u00a0Only you can judge; but it\u2019s easy to fool yourself, and perceive patterns that don\u2019t stand up to scrutiny. \u00a0Your best strategy is to keep meticulous daily records. \u00a0Rate your pain or your energy on a scale from 1 to 10. \u00a0How many hours were you able to work undistracted? \u00a0Record, too, brief, qualitative descriptions of your mood and energy level and your creative output. \u00a0Don\u2019t review these until the experiment is complete, and then look back and evaluate your record as objectively as you can, comparing \u201con treatment\u201d and \u201coff treatment\u201d times in the aggregate. \u00a0(For this stage, you may find it helpful to engage a friend to evaluate your written account more objectively.)<\/li>\n<li style=\"font-weight: 400\">Yet more subtle are indications from our bodies that offer a clue to what is good or bad for us. \u00a0Many of us have been raised in a culture which tells us to pop an aspirin and show up at work no matter how miserable we feel. \u00a0Yoga and meditation practice can lead you back to an intuitive sensitivity to your body. \u00a0Mindfulness practice can re-sensitize you to subtle body signals that you have learned to ignore.<\/li>\n<\/ul>\n<p>Blood tests are useful for longer-term experiments. \u00a0A1C is an indication of average blood sugar. \u00a0C-reactive Protein tracks with the body\u2019s inflammation level and correlates with cardiovascular risk.<\/p>\n<p>Many of us are interested in long-term benefits that are unlikely to show up in a two-week test. \u00a0These experiments are not more\u00a0difficult, just slower. \u00a0Test at intervals of 6 months to a year. \u00a0There are commercial telomere tests available that can plot one indication of your biological age. \u00a0Horvath\u2019s DNA methylation clock can be used to track a set of markers that is, collectively, a\u00a0more robust, objective indicator. \u00a0For these long-term tests, we don&#8217;t have the luxury of trying one test at a time, (one year on &#8211; one year off?) but\u00a0we can try a combination of lifestyle changes based on a hunch, then check our progress annually to see if we are on the right track.<\/p>\n<p>\u00a0<\/p>\n<p><strong>What to try?<\/strong><\/p>\n<ul>\n<li>Do I sleep better if I do Qi Gong before bed?<\/li>\n<li>Do I get more done at work if I bicycle than if I drive?<\/li>\n<li>Does my blood sugar go down if I don&#8217;t eat wheat?<\/li>\n<li>Is daily aspirin making my back less stiff?<\/li>\n<li>Does bacopa improve my creative output?<\/li>\n<\/ul>\n<p>A friend of mine, a PhD biochemist, jumped into action when his sister-in-law was diagnosed with glioblastoma. \u00a0This is a devastating form of brain cancer that is almost always fatal within a few months. \u00a0But, scouring PubMed, Greg located three unrelated, obscure studies from the past in which safe and available supplements led to complete cures in a minority of subjects. He found the three ingredients\u2014one from an off-shore source\u2014but he couldn\u2019t get permission to administer them until his sister-in-law was in a hospice unit, deemed to be in her final hours. \u00a0But within weeks she was back on her feet, and now, two years later, she is still alive.<\/p>\n<p>This is one model: look on the web for listserves with personal stories, or traditional medicines, or \u201cfailed\u201d trials in the medical literature. \u00a0<\/p>\n<p>I\u2019ve mostly used <a href=\"https:\/\/examine.com\/\">Examine.com<\/a> and <a href=\"http:\/\/www.lifeextension.com\/\">LEF<\/a> as fertile sources of ideas. \u00a0Suggestions from friends are\u00a0perfectly\u00a0legitimate. \u00a0Follow your intuition;
