We should begin with hypotheses about what we are evaluating. \u00a0My idea is to consult some experienced experts, and also to crowd-source this choice, and to ask for your help in selecting the supplements and life habits to be evaluated.<\/span><\/i><\/p>\n<hr \/>\n<p><span style=\"font-weight: 400\">Details of the trial were described in two blog posts last spring [<\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2018\/04\/08\/the-mother-of-all-clinical-trials-part-i\/\"><span style=\"font-weight: 400\">One<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2018\/04\/17\/the-mother-of-all-clinical-trials-part-ii\/\"><span style=\"font-weight: 400\">Two<\/span><\/a><span style=\"font-weight: 400\">] and a more <\/span><a href=\"https:\/\/www.researchgate.net\/publication\/324946537_An_Incipient_Revolution_in_the_Testing_of_Anti-aging_Strategies?_sg=Jv2AVOs8kPW7M1m4TB-vTDWTLXXbNd6rumTj_VKYmxbQKRt1O_-1PN0tALN6jjyGXFWal5PX9ba8l5r6bWs-FEvbvAdo3MjTw-OOFV3y.gOs6QrNKEKGhNJNueHvNrKKfAUIeh9N1Qb75jX-PkT-rfhkZQVCFMy2BgjQatvwPfhOoRhplsp1FkrPLrTRCuw\"><span style=\"font-weight: 400\">technical manuscript<\/span><\/a><span style=\"font-weight: 400\"> submitted in May. \u00a0Outcome will be evaluated based on a variant of <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5940111\/\"><span style=\"font-weight: 400\">DNA PhenoAge<\/span><\/a><span style=\"font-weight: 400\">, taken from a blood test before, amid, and after the two-year trial. \u00a0We use methylation pattern differences rather than mortality or health outcomes because the latter take a long time to reveal themselves, and make anti-aging trials prohibitively expensive. \u00a0Using methylation clocks as an endpoint is a new idea, and we don\u2019t know if it will work, but if it does, it will be 100 times cheaper and 10 times faster than previous methods. We will have enough bandwidth to test a dozen different measures at once, which itself is a revolutionary step. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Many measures are known that are thought to increase life expectancy by a year or a few years each. \u00a0Of course, we want to know which ones offer their greatest benefits. But even more important, we want to know how they interact, synergize, and interfere with one another. \u00a0If any one of these measures offered major benefits\u2014say 20 years of life\u2014its effects would be so apparent that we would probably know it already. Likewise, if these measures added up to 20 years of extra life, we would all know some people who are obviously younger than their chronological age. \u00a0Realistically, we must assume that most of the things we do are redundant. Combining <\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2012\/11\/26\/is-metformin-an-anti-aging-drug\/\"><span style=\"font-weight: 400\">metformin<\/span><\/a><span style=\"font-weight: 400\"> with <\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2015\/10\/13\/can-botanicals-replace-metformin\/\"><span style=\"font-weight: 400\">berberine<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2015\/10\/13\/can-botanicals-replace-metformin\/\"><span style=\"font-weight: 400\">gynostemma<\/span><\/a><span style=\"font-weight: 400\"> may offer little additional life expectancy compared to any one separately. \u00a0A panoply of different anti-inflammatory strategies may be little improvement over an aspirin a day.<\/span><\/p>\n<p><span style=\"font-weight: 400\">But we hope there are exceptions. \u00a0If two different measures act via completely different metabolic pathways, we have reason to hope that their effects should compound. \u00a0For example, perhaps life extension interventions based on <\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2017\/07\/25\/mitochondria-in-aging-ii-remedies\/\"><span style=\"font-weight: 400\">mitochondrial health<\/span><\/a><span style=\"font-weight: 400\"> synergize\u00a0with interventions based on <\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2017\/02\/24\/fasting-mimicking-diet-can-you-make-it-a-habit\/\"><span style=\"font-weight: 400\">rebooting the immune system<\/span><\/a><span style=\"font-weight: 400\">. \u00a0Then we might hope that the life extension available from these two measures together is greater than the sum of what we get from the two separately.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><b>Study design<\/b><\/p>\n<p><span style=\"font-weight: 400\">We will not tell the people who sign up for this study what to eat, what pills to take, or how much to exercise. \u00a0We will ask people what they are doing, what they are eating, and what supplements they are taking in a detailed questionnaire. \u00a0We will select subjects so as to represent a broad array of different strategies and different combinations among these strategies.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Broad, but not too broad. \u00a0We will have enough statistical power to estimate the interactions among every pair of measures out of 12 that we take as our independent variables. \u00a0These 12 should be chosen in advance, so the study has a clear focus. If there are more than 12, the number of interactions increases rapidly beyond what we can hope to distinguish (with multivariate statistics). \u00a0I\u2019ve decided to start with 15, and winnow the list as people sign up for the study and we see what <\/span><\/p>\n<p><span style=\"font-weight: 400\">Here are the criteria I propose:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Each measure, separately, should have either human mortality data or longevity data to back it up.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The measures should be easily available to all (excluding intravenous drugs or transfusions)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The measures should be well-enough known that they are already in common use (and we will have no trouble identifying a diverse group of subjects who use them)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">I find that it\u2019s hard to limit the list to 15. \u00a0It may make sense to include a few measures that are so well established that every participant will be required to comply in order to be included in the study. \u00a0In this category, we might put<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Non-smoking<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Limited alcohol consumption (or none)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Vitamin D at least 5,000 IU daily <\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Multi-mineral supplement with magnesium, zinc, chromium, and selenium<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Exercise equivalent to minimum 5 hours a week of walking or yoga<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">Body weight (BMI) is an important longevity factor, but difficult to account. \u00a0Studies show that maximum lifespan is associated with BMI between 21 and 25, but <\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2012\/12\/03\/you-cant-be-too-rich-or-too-thin\/\"><span style=\"font-weight: 400\">in my interpretation<\/span><\/a><span style=\"font-weight: 400\">, lower BMI is always beneficial for any given individual. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\"><img loading=\"lazy\" decoding=\"async\" class=\"\" src=\"https:\/\/protonsforbreakfast.files.wordpress.com\/2012\/07\/bmi-mortality-versus-age.jpeg?w=450&amp;h=226\" alt=\"BMI Mortality versus age\" width=\"544\" height=\"280\" \/><\/span><\/p>\n<p><span style=\"font-weight: 400\">The reason for the apparent paradox is that individuals have genetic disposition to be overweight or underweight. \u00a0Those who are genetically underweight tend to overeat, because they can do so with no social stigma. Those who are genetically overweight feel compelled to diet all the time (women more than men), and they may be restricting calories just to keep their BMI at 25. \u00a0These dieters get the most benefit from Caloric Restriction, despite the fact that they don\u2019t look thin.<\/span><\/p>\n<p><strong>List of things that lengthen your life<\/strong><\/p>\n<ol>\n<li><b>Love.<\/b><span style=\"font-weight: 400\"> \u00a0Men who are married or in close relationships have 7% lower mortality than singles. \u00a0The number is 4% for women [<\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S016762960400061X\"><span style=\"font-weight: 400\">ref<\/span><\/a><span style=\"font-weight: 400\">]. \u00a0These numbers correspond to less than a year of life expectancy. \u00a0A <\/span><a href=\"https:\/\/www.cambridge.org\/core\/services\/aop-cambridge-core\/content\/view\/0374DD86EE9D6975FA848FF6C05C6F48\/S0007125000244590a.pdf\/div-class-title-impact-of-loneliness-and-depression-on-mortality-results-from-the-longitudinal-ageing-study-amsterdam-div.pdf\"><span style=\"font-weight: 400\">different study<\/span><\/a><span style=\"font-weight: 400\"> finds loneliness increases mortality by 50%, corresponding to almost 5 years of life.<\/span><\/li>\n<li><b>Empowerment:<\/b><span style=\"font-weight: 400\"> Staying employed is worth <\/span><a href=\"https:\/\/europepmc.org\/abstract\/med\/12785422\"><span style=\"font-weight: 400\">up to 14 years<\/span><\/a><span style=\"font-weight: 400\">, and I like to think this is more about being needed than making money. \u00a0<\/span><a href=\"http:\/\/jech.bmj.com\/content\/66\/7\/573.short\"><span style=\"font-weight: 400\">This study<\/span><\/a><span style=\"font-weight: 400\"> claims that the big difference is wealth.<\/span><\/li>\n<li><b>Anti-inflammatories<\/b><span style=\"font-weight: 400\">: Aspirin, ibuprofen, curcumin, or fish oil. \u00a0<\/span><a href=\"https:\/\/www.bmj.com\/content\/352\/bmj.i1548\/\"><span style=\"font-weight: 400\">This study<\/span><\/a><span style=\"font-weight: 400\"> attributes about a year of life expectancy to daily aspirin.<\/span><\/li>\n<li><b>High fiber<\/b><span style=\"font-weight: 400\">, a proxy for healthy gut flora. \u00a0We know that they are important, but don\u2019t yet know how to manage the biota for maximal life expectancy.<\/span><\/li>\n<li><b>Vegetable-based diet <span style=\"font-weight: 400\"><a href=\"http:\/\/ajcn.nutrition.org\/content\/78\/3\/526S.short\">This review<\/a> concluded that vegetarians live 3 years longer, but methodologies and results vary considerably.<\/span><\/b><\/li>\n<li><b>Meditation <\/b><span style=\"font-weight: 400\">This is difficult to evaluate, and data is unreliable but encouraging [<\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2211376\/#R20\"><span style=\"font-weight: 400\">ref<\/span><\/a><span style=\"font-weight: 400\">]. \u00a0The only <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S030645301000243X\"><span style=\"font-weight: 400\">careful study<\/span><\/a><span style=\"font-weight: 400\"> linked meditation not to mortality but to telomerase activity. \u00a0I confess I am including meditation in the list from my own intuition and experience. \u00a0<\/span><\/li>\n<li><b>Intermittent fasting <span style=\"font-weight: 400\">Extends lifespan in mice and lowers mortality in nursing home studies [<\/span><a href=\"http:\/\/www.anti-aging-plan.com\/en\/scientific_evidence\"><span style=\"font-weight: 400\">ref<\/span><\/a><span style=\"font-weight: 400\">].<\/span><\/b><\/li>\n<li><strong>Interval training<\/strong> <span style=\"font-weight: 400\">Reputed to be the most efficient path toward cardiovascular fitness [<\/span><a href=\"http:\/\/journals.plos.org\/plosone\/article?id=10.1371\/journal.pone.0154075&amp;__hstc=193161458.205442a1e835c34ab7bf88e972fdd7a7.1466294400033.1466294400034.1466294400035.1&amp;__hssc=193161458.1.1466294400036&amp;__hsfp=3105786926\"><span style=\"font-weight: 400\">ref<\/span><\/a><span style=\"font-weight: 400\">], and there is limited documentation of benefit for all-cause mortality [<\/span><a href=\"https:\/\/physoc.onlinelibrary.wiley.com\/doi\/pdf\/10.1113\/JP271041\"><span style=\"font-weight: 400\">ref<\/span><\/a><span style=\"font-weight: 400\">]. <\/span><\/li>\n<li><b>Donating blood <span style=\"font-weight: 400\">This is another quirky inclusion on my part, but there is data to support it, which <\/span><a href=\"https:\/\/scienceblog.com\/joshmitteldorf\/2015\/11\/09\/does-donating-blood-extend-your-life-expectancy\/\"><span style=\"font-weight: 400\">I reviewed<\/span><\/a><span style=\"font-weight: 400\"> a few years ago. \u00a0<\/span><\/b><\/li>\n<li><b>NAC<\/b><span style=\"font-weight: 400\"> just <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26039957\"><span style=\"font-weight: 400\">one study<\/span><\/a><span style=\"font-weight: 400\"> \u2014 30% increase in lifespan of mice<\/span><\/li>\n<li><b>DHEA<\/b><span style=\"font-weight: 400\"> Lower blood levels of DHEA are clearly associated with greater age and higher mortality at the same age, but the direction of causality is in dispute.<\/span><\/li>\n<li><b>Metformin <span style=\"font-weight: 400\">Prescribed for diabetes for decades, this drug also lowers mortality from cancer and heart disease [<a href=\"https:\/\/www.sciencemag.org\/news\/2015\/09\/feature-man-who-wants-beat-back-aging\">planned clinical trial<\/a>].<\/span><\/b><\/li>\n<li><strong>Rapamycin<\/strong> <span style=\"font-weight: 400\">The most convincing data available for any supplement for life extension in mice. \u00a0Early adopters are beginning to experiment on their own.<\/span><\/li>\n<li><b>Quercetin + Dasatinib<\/b><span style=\"font-weight: 400\"> (or other senolytics). \u00a0Senolytics are the best near-term hope we have for a breakthrough in anti-aging medicine; but the combination of quercetin +dasatinib is not yet discriminating enough to be safe for humans, meaning it kills too many regular cells.<\/span><\/li>\n<li><b>Epithalamin <span style=\"font-weight: 400\">\u00a0Very promising data from Russia [<\/span><a href=\"https:\/\/link.springer.com\/article\/10.1007\/s10522-009-9249-8\"><span style=\"font-weight: 400\">ref<\/span><\/a><span style=\"font-weight: 400\">], both in rodents and in people, but there is no one trying to reproduce them in the West.<\/span><\/b><\/li>\n<li><b>Ashwagandha<\/b><span style=\"font-weight: 400\"> [<\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3252722\/\"><span style=\"font-weight: 400\">many benefits, but no mortality or rodent lifespan data<\/span><\/a><span style=\"font-weight: 400\">] <\/span><\/li>\n<li><b>Selegiline (deprenyl)<\/b><span style=\"font-weight: 400\">. \u00a0In classic studies from the 1980s, lifespan of rats was extended. \u00a0Data in humans is contradictory [<\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/j.1527-3458.2001.tb00202.x\"><span style=\"font-weight: 400\">ref<\/span><\/a><span style=\"font-weight: 400\">].<\/span><\/li>\n<\/ol>\n<p>Perhaps we should begin with a guess about which combinations are likely to be highly redundant. \u00a0In this way, we could cluster together different strategies and condense more strategies into a manageable list. \u00a0It might look like this:<\/p>\n<ol>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Anti-inflammatories<\/strong> (aspirin, ibuprofen, statins, omega 3, curcumin, boswellia)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Blood sugar control<\/strong> (metformin, berberine, gynostemma, chromium)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Social factors<\/strong> (family, employment, wealth, community support, marriage, sex, communing with nature, empowerment) <\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Mitochondrial supplements<\/strong> (NAC, CoQ10, PQQ, NR, melatonin, glutathione, carnosine, ALA)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Immune support<\/strong> (reishi mushrooms, cistanche, andrographis, goldenseal, echinacea)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Adaptogens<\/strong> (rhodiola, ashwagandha, bacopa, silymarin, pycnogenol)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Telomerase activators<\/strong> (silymarin, astragalus, ashwagandha, horny goat weed) <\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Senolytics<\/strong> (quercetin, dasatinib, fasting)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Diet<\/strong> (everything from high-protein to high fiber to vegan to paleo in one cluster?)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Limiting and intermittency of diet<\/strong> (long- and short-term fasting, CR, BMI)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Exercise<\/strong> (aerobic, strength, interval, walking, competitive sports, yoga)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Mental focus<\/strong> (meditation, prayer, yoga, tai chi, spiritual practice, cafeine)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Neuroprotective<\/strong> (ashwagandha, rhodiola, ginkgo, melatonin, bacopa, selegiline, gotu kola, coffee, tea, blueberries, chocolate)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Multivitamin supplements<\/strong> (including mega D, mega-C, B12, carotenoids and tocopherols)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Sex and steroid hormones<\/strong> (DHEA, prostaglandin, progesterone, SAMe, testosterone)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>Angiotensin inhibitors<\/strong> (Lotensin, captopril, enelapril)<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400\">Or\u2014the best of both worlds\u2014we might structure the study in such a way that it can be analyzed after the fact either with individual strategies or clusters of strategies.<\/span><\/p>\n<figure id=\"attachment_746\" aria-describedby=\"caption-attachment-746\" style=\"width: at the end. \u00a0I imagine I am two years down the road, taking my first look at results from 5,000 life-extenders. \u00a0The first thing I will want to do is to look for outliers. Are there a few people who stand out from the bell curve, aging much more slowly? \u00a0If so, what do they have in common? The advantage of this approach is that it gives maximal flexibility in telling us exactly what we most want to know. \u00a0The disadvantage is that it is easy to fool ourselves and imagine patterns in a small set of random errors. When there is no initial hypothesis, there is no objective way to calculate a probability that what we find is the result of chance.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Our null hypothesis is that there are no systematic outliers, but only a smooth tail to the probability curve. \u00a0If there are a few scattered individuals in 5,000 who are aging much more slowly than the rest, we will not find any common thread in what they are doing, and so we must explain their data as anomalies or mistakes. \u00a0If this is our result, it will be disappointing, sobering, but liberating as well.
