Immune competence declines with age, yet people over 50 become prone to giant cell arteritis (GCA), an autoimmune vasculopathy. Four aging-related processes drive this risk: (1) arterial wall aging generates abundant neoantigens, (2) aging makes the blood–tissue barrier leaky and activates endothelial cells to stimulate T cells, (3) persistence of “immune youth” maintains long-lived, self-renewing CD4+ T cells that fuel pathogenic responses, and (4) prolonged immune youth prevents T cell exhaustion by removing checkpoint inhibition. This delayed T cell aging sustains memory responses to tissue-derived neoantigens.
